What To Do When You Receive a Jury Summons

Step 1: Complete the jury qualification questionnaire.

If you received a summons in the mail from the U.S. District Court for the District of Colorado, you are required by law to complete the questionnaire form within ten days of receipt. The questionnaire is available online at www.cod.uscourts.gov by clicking on the green eJuror icon, or you may call 800-359-8669, option 2, to request a hard copy of the questionnaire be mailed to you. If you are unable to fill out this form, someone else may do it for you, provided that person indicates in the “Remarks” section why it was necessary for him or her to do so for you. If you do not return the completed questionnaire form within ten days, you may be summoned to report to court, at your own expense, to complete the questionnaire.

Step 2: There are two ways to check your reporting status: online or by phone. You will need your 9-digit participant number located on the jury summons. The summons states the date(s) to check, and the time your message will be updated.
